Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. We accept most major insurances including: Medicare, Tricare, Veterans Choice, Blue Cross/Blue Shield, United Health Care, Humana, Cigna and Medicaid. Please check with your insurance company regarding your physical therapy benefits, authorization and referral requirements, deductible, coinsurance and copays and limits on number of visits per year.

Cash, check and most major credit cards accepted for payment.
Cancellation Policy
We ask for at least 24 hour advanced notice. If you have repeated short notice cancellation, or do not attend scheduled appointments, you may be removed from the schedule.
